OETAS > Registration, Fees, Refund/Credit Policy and Inclement Weather
REGISTRATION
An application for registration of courses can be found on the last page of this catalogue. You may also download application forms from our web site at https://maryland-adaa.org/catalogdownloads.html. To do this, look to the left of the web page for the OETAS link to download the catalog and application. Fill out the application right on the web site, print it out and mail your application with payment to the OETAS office. Applications without full payment cannot be accepted. Only agencies paying by R*STARS transfer may FAX applications with the CUR DOC number. You will receive confirmation, including directions to the training by e-mail. For those applicants without email, a confirmation will be mailed to the address provided on the application. If you do not receive confirmation concerning your registration, you are not registered. Contact the OETAS office prior to class attendance. Only checks, money orders or R*Stars Transfers are accepted as payment. Purchase orders will be accepted from Federal Agencies only.
INCLEMENT WEATHER POLICY
In the event of inclement weather, snow or ice, please check the ADAA web site at http://dhmh.maryland.gov/adaa/ for the delay or cancellation of scheduled training. Postings of delayed opening or cancellation will be made by 7:00 a.m. on the date of the scheduled class. Those who do not have internet access, may call the OETAS office at after 7:00 a.m. on the date of the scheduled class for information concerning the delay or cancellation of class.
FEES
Twenty-seven hour course: 9:00 a.m. to 4:00 p.m. - (4 Day Course) 27 CEU - $200.00
Twenty hour course: 8:30 a.m. to 4:30 p.m. - (3 Day Course) 20 CEU - $160.00
Twelve hour workshop: 9:00 a.m. to 4:00 p.m. - (2 Day Workshop) 12 CEU - $120.00
REFUND/CREDIT POLICY
A refund/credit will be granted if a written request with the applicant’s social security number is received at the OETAS Catonsville office no later than five working days prior to the first class session. Refunds will be sent to the address on your application, unless specified otherwise. Please allow 3-4 weeks for refunds to arrive in the mail. Credits will be valid for one year from the original class. This form can be downloaded from https://maryland-adaa.org/OETASForms.html under the OETAS link. Please allow 6-8 weeks for refunds to arrive in the mail.
